2 months	DTaP Hep B Hib IPV PCV Rotavirus
4 months	DTaP Hep B Hib IPV PCV Rotavirus
6 months	DTaP Hep B Hib IPV PCV Rotavirus
6 months + (yearly)	Flu Shot -Children under 9 years of age receiving the flu vaccine for the first time are recommended to have 2 doses one month apart.
12 months	Hep A MMR Varicella
12-15 months	Hib PCV
12-18 months	DTaP
18 months	Hep A -At least 6 months after the first dose.
4-5 years	DTaP IPV MMR Varicella
10+ years	Meningococcal (Men B) (if local outbreak; absent or damaged spleen; complement deficiency)
11 years	Meningococcal ACWY
11-12 years	HPV (Gardasil) Tdap
15 years	HPV (Gardasil) (if started at 15 yrs, 3 doses- 2nd shot: 2 months after first shot)
15 years	HPV (Gardasil) (if started at 15 yrs, 3 doses- 3rd shot: 6 months after first shot)
16 years	Meningococcal ACWY
16-18 years	Meningococcal (Men B) (Ask us about frequency)
